Your First Visit

Your initial appointment is typically directed towards obtaining current medical and dental information. Radiographs (X-rays) and any other diagnostic information will enable the doctor to provide a thorough diagnosis and the ability to generate treatment options during your consultation.

Treatment may sometimes be performed the day of your initial visit. A complex medical/dental history or an involved treatment plan may, however, require further evaluation or even a second appointment to begin treatment on another day.

Please assist us by providing the following information at your first visit:

  • X-rays if available
  • A list of all current medications
  • If you have dental insurance (Awesome for you! We are not kidding!), please bring the necessary completed forms with you. This can save you time and allows us to more rapidly process your claim.
  • Completed patient registration form and health/dental history form-We’ll mail these forms to you if you’d like and there is sufficient time between your calling us and your initial visit! We look forward to your ability to being able to download these forms.
  • IMPORTANT: A PARENT OR GUARDIAN MUST ACCOMPANY ALL PATIENTS UNDER 18 AT THE INITIAL VISIT!
